2018-2019 RE-REGISTRATION FORM

A new form is being prepared for you to complete!

Kindergarten

In order to register your child for kindergarten, the following items MUST be presented at this time:

a.) Birth Certificate (child must be five years of age by September 1, 2017)

b.) Baptismal Certificate

c.) Complete Immunization Records

Grades 1 through 8
At this time, registration for new students in grades 1 through 8 will be accepted on a space-available basis. Please bring a copy of student’s report card and recent standardized test results. If student does not attend Catholic school, please bring proof of sacraments.

(The tuition schedule for 2087-19 will be available after January 16, 2018.)
